WildPlay Wavier - ACTION REQUIRED !!

Some important information regarding the Class of 2022 dry grad activity …

Attached is the SD78 Hope Secondary School parent permission form, along with the WildPark wavier form. These forms are required to be filled out and returned to the office by May 24th, 2022.

Please note, the wavier form has highlighted areas to show the fields that must be completed. Just a few notes on completing the waiver, the box on the second page is meant for an initial NOT a check mark, and the upper personal information section is meant for the participant's information, not the parents. I encourage you to read the document carefully, but please do not alter anything as this will prevent WildPlay from accepting it. You will not be able to participate with out this parent/guardian signed document. Alterations include crossing out, removing, or adding text in any way, which will invalidate the wavier.

All grad fee payments, permission forms, waivers, digital or paper copy, MUST be completed by May 24th. Paper copies, SD78 permission form and WildPlay are to be submitted to the office.
